I really don't know what causes them, but mine were so bad that I had to see an ENT (ear, nose, throat) specialist for years. It can also be related to your sinuses--all that stuff is connected somehow, I guess that's why they call those doctors ENTs. Best to get it taken care of ASAP. They'll either give you anti-biotics or ear drops. Maybe both. Thank goodness it's been years since I had a major one. Tossing a couple cents on top of Abdul's comment, excess water left in your ear canal (showering, bathing, what have you) can combine with some nasty stuff that just happens to be in there already. It got so bad with me when I was young that I would spend a touch of time using a blowdryer to lightly dry out my ear after showering... Anywho, best of luck at the doctor.
